Job Description :

Carpenter's Place (CP), a not-for-profit faith-based agency, has an opening for a full-time general case manager. This candidate must be comfortable working in a setting with individuals experiencing homelessness, addiction and/or mental health issues. Must possess good verbal and written communication skills, be detail oriented, be a team player with high energy, and have the ability to be flexible while maintaining professionalism in an ever-changing environment.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Work with CP guests in developing and executing an action plan/Life Recovery Plan.
  • Assist and identify any potential participants to that could benefit from CP internal services and transitional housing opportunities.
  • Assist guests in transitioning from homelessness or transitional housing to their own housing
  • Coordinate and maintain positive working relationships with landlords and potential landlords
  • Perform Case Work with Guests in the following areas:
    • Intakes - Perform a comprehensive psychosocial assessment that addresses the patient's physical and psychosocial health, environment, support systems, educational needs, and financial resources.
    • Assessments - determine areas of highest need.
    • Action Plans - determine goals with each resident that are tailored to his/her identified needs and help them to implement an Action Plan.
    • Referrals & linkage - refer to outside agencies as appropriate and provide advocacy as needed to ensure interagency collaboration.
    • Follow-up - confirm implementation of Action Plan steps by the resident and referral agencies.
  • Review Life Recovery plan with Guests periodically to determine progress and/or need for modifications in the Plan.
  • Assist guests with communicating with outside service agencies to insure proper implementation of services.
  • Follow-up with Guest and agency as required to confirm proper implementation of services.
  • Resource investigation and crisis intervention as required.
  • Maintain documentation of interactions, advocacy, referrals, goals and outcomes and maintain accurate statistical and client records.
  • Act as liaison between court services and clients when appropriate.
  • Maintain files and all required data as determined by HUD and Carpenter's Place.
  • Provide community education where appropriate.
  • Develop and maintain current information on availability of referral sources and linkages.
  • Develop a supportive, professional relationship with guests.
  • Provide transportation to guests to community services as needed.
  • Be a team player, working with the Director of Guest Services and Case Management Team
  • Other duties as requested by Director of Program Services

Education/Experience: We will consider candidates with a Bachelor's degree in Human Services, Social Work, or related field is required. Mental health and addiction services experience preferred. A valid driver's license with reliable transportation is required.
