Job Description :

Albany Medical Center is seeking a dedicated and compassionate Bariatric Nurse Navigator to join our Surgery Bariatric team. This role is vital in guiding our patients through their bariatric journey, ensuring they receive the highest quality care and support.

In this position, you will work closely with physicians and advanced practice providers to deliver direct patient care. You will be responsible for patient triage (both in-person and telephonic), assessment, planning, and evaluation of personalized care plans.

Key Responsibilities:

  • Accurate assessment and recording of patient vital signs, reporting any abnormal findings to the treating provider.
  • Documenting comprehensive patient histories—including Medical, Surgical, and Social information—as well as conducting focused physical assessments in line with clinic procedures.
  • Administering medications and treatments to patients as directed by a physician or APP.
  • Assisting in office procedures related to bariatric care under the supervision of licensed medical professionals.
  • Reviewing and triaging incoming test results, patient requests, and pharmacy renewals, ensuring timely follow-ups.
  • Facilitating the utilization of resources to achieve optimal patient outcomes while contributing to quality metrics.
  • Delegating appropriate tasks to licensed practical nurses or medical assistants as per their scope of practice.
  • Ensuring compliance with regulatory agencies and internal policies, including The Joint Commission standards.
  • Participating in the orientation and education of newly hired staff.
  • Engaging in continuous education to maintain your expertise in bariatric nursing and patient care practices.
  • Providing educational support to patients about their visits and overall health goals.

Minimum Qualifications:

  • Associate's degree in a state-accredited Registered Nursing program.
  • Current New York State RN licensure.
  • Current CPR certification.
  • Certification in a relevant specialty, particularly in an ambulatory setting, is preferred.
  • Exceptional customer service and communication skills with the ability to effectively interact with patients, families, and team members.
  • Proficient in computer software, specifically MS Word and Excel; experience with electronic health records is advantageous.
  • Ability to administer medications via all routes and calculate appropriate dosages.
