Center for Human Development, Inc. is seeking a full-time Family Advocate to work independently under the guidance of the Babies First!/CaCoon Nurses. The Family Advocate visits prenatal moms, children 0 to 5 years and children with disabilities 0-21 years. This position works with qualifying families who have identified risk factors, partnering with the family to access and utilize resources while encouraging healthy growth and development. The Requirements:
- Bachelor's Degree in Human Services or related field: or combination of three years education, and experience in public health, mental health, alcohol and drug, health occupations, or health education.
- Ability to develop and implement individual and group skills training programs.
- Ability to work effectively with a team, other professionals, and children and families.
- Ability to organize and facilitate groups.
- Must possess a valid Oregon Driver's license and be insurable for client service purposes.
- Must pass all criminal history check requirements and drug screenings.
The Essential Functions:
- Provide case management.
- Work independently to follow a care plan with goals, objectives, and activities to meet family needs and program requirements.
- Establish and maintain a trusting relationship with families by providing regular contact through home visits.
- Develop and maintain a referral network so referrals can be made quickly and effectively to appropriate agencies.
- Assist parents in making and attending appointments with doctors, social service agencies, etc.
- Act as an advocate for families with other community agencies.
- Prepare and maintain timely documentation and regular communication with Registered Nurses.
- Assist families with paperwork needed in applications for resources such as medical, housing or financial assistance.
- Provide data collection and evaluations for program tracking to state and county.
- Work effectively with diverse families.
- Work with WIC coordinator to assist families with in-home certifications, which includes taking height, weights and checking hemoglobin according to WIC state regulations
- Attend regular staff meetings, team meetings, in-service trainings and other meetings as required.
- Attend both Babies First!/CaCoon and WIC trainings.
- Other duties as assigned.
