Work Schedule: Full time, Monday through Friday 8 am to 4 pm (40 hours), with flexibility to meet program needs
A Day in the Life of a Care Manager:
As a dedicated full-time Care Manager, you will play a pivotal role in providing exceptional and comprehensive care management services to adults. You will be responsible for maintaining contact with members throughout Wayne, Monroe, Ontario and Seneca counties.
Provides assistance with housing, transportation, food, clothing and other barriers.
Provides self-management support, health promotion, connection/referrals to providers, community based organizations, social supports, transitions of care and crisis support with the goal of decreasing barriers to attending appointments and following the plan of care
Provides coordination with Primary/Specialty medical care, acute and outpatient medical, mental health, substance abuse services, and other care managers involved in supporting the client
Provides crisis intervention, addressing events such as emergency department visits or inpatient or other crisis events to assure interventions are effective and necessary modifications to plan of care are made
Home and field visits to provide counseling, crisis intervention referral and advocacy services, coordination of services and assistance with basic needs
Who We Are Looking For:
Bachelor's Degree in human services or related field or New York State License Registered Nurse
Two years' relevant experience providing direct services to person diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse
Computer experience with working knowledge of Microsoft Office programs including but not limited to Excel, Outlook and Word.
Must possess a current, valid New York State driver's license that meets the Agency driver standards
