As a Care Coordination Manager, you can develop a team and have a positive impact on patient experience at Neighborhood. About the Role: As Care Coordination Manager at Neighborhood, you'll manage a team of care coordinators, using your experience to identify, encourage and develop the skills and talents of your employees so they can better serve our patients. You will provide input to senior leaders for training, and implement workflows within your department. You'll also collaborate with other departments and be involved with quality improvement efforts. Responsibilities include:

  • Overseeing the daily operations and performance of the care coordination team
  • Reporting and tracking of care coordination metrics
  • Implementing and maintaining department policies and procedures
  • Collaborating with cross functional management teams to improve quality efforts
This position is not based at one site. You'll be traveling to all Neighborhood sites in Western New York as needed. Education and Skills to be a Care Coordination Manager:
  • Bachelor's degree or 3-5 years in a relatable field
  • Maintain CPR and licensure(s)/certifications required to perform job duties
  • Excellent oral and written communication skills
  • Read, write, and speak the English language. A second language of Spanish is preferred

  • Teamwork; you are open to others' unique perspectives, and will collaborate to meet shared goals
What We Offer: Compensation: $75,900 - $78,000 annual salary (based on a full time, 40 hour work week) Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications, and other position related components.
