You are applying for a position through Medix, a staffing agency. The actual posting represents a position at one of our clients. Job Summary Our client is seeking a Care Coordinator to manage and integrate physical health, mental health, and substance use services for adults with significant behavioral health needs. This role involves working under the NYS DOH Health Home Model to provide comprehensive care management, including conducting assessments, developing service plans, and facilitating access to Home and Community-Based Services (HCBS) not covered by traditional Medicaid plans. Key Responsibilities

  • Gather information, conduct outreach, intake, assessment, and reassessments.
  • Provide care management through coordination of medical, mental health, HCBS, and substance abuse services.
  • Conduct assessments and prepare comprehensive care plans as per NY State and Managed Care Organizations.
  • Collaborate with the individual's HARP team, including MCOs and HCBS providers.
  • Ensure entitlements, insurance, and benefits are in place and maintained.
  • Develop service plans and resolve barriers to effective service utilization.
  • Monitor clients' progress and ensure follow-up with medical directions.
  • Escort clients to appointments as needed and document service information in databases.
  • Assist in crisis intervention and provide or refer to crisis services.
  • Perform extensive field work, including home visits and community engagements.
  • Ensure clients follow-up with aftercare discharge activities.
Qualifications
  • Bachelor's Degree Required; Masters Degree Preferred in Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Physical or Recreational therapy, or related fields.
Experience
  • B.A. level candidates: Two (2) years of related experience working with individuals with severe mental illness.
  • M.A. level candidates: One (1) year of similar experience.
Skills
  • Excellent interpersonal and time management skills.
  • Proficiency in email and documentation on electronic platforms.
  • Comfortable with fieldwork and navigating social services systems.
  • Ability and willingness to travel regularly with clients to various locations using safe transportation modes.
Additional Requirements
  • Knowledge of Medicaid, Social Security, and other entitlements is preferred.
  • Bilingual/Spanish skills are a plus.
