Maimonides is an affiliate of Northwell Health and a major clinical training site for SUNY Downstate College of Medicine. Overview We are seeking a Patient Engagement Associate to join our team. Responsibilities
  • Track and analyze data and reports from different sources such as medication adherence reports and patient charts
  • Perform phone outreach based on patient registries and gaps in care reports for medication adherence refill reminders and refill requests.
  • Collect medication histories utilizing different sources such as electronic medical records and pharmacy technology platforms to assist pharmacists with medication reconciliations.
  • Communicate with community pharmacies to assist patients and providers with medication refills, medication reconciliation, and troubleshooting access to medications.
  • Engage patients to participate in self-management and gap closing activities, as measured under value-based contracts including upside and downside arrangements.
  • Maintain records of all outreach activity on each member/patient.
  • Provide support to pharmacists and other clinical personnel in the ambulatory clinics and community offices.
  • Update and maintain appropriate documentation related to medication outreach in patient electronic medical records.
  • Prepare reports for individual providers of outreach activities to their individual assigned members.
  • Develop relationships with providers and other clinical staff to facilitate effective communication and performance improvement.
  • Participate in routine meetings discussing opportunities for quality and process improvements.
Qualifications
  • High School diploma or equivalent. Bachelor's degree preferred
  • Certification as Pharmacy Technician by the National Technician Board OR enrollment in an approved Pharmacy Technician program required OR Enrollment in a medically-related training program (e.g., RN, LPN)
  • Experience in community pharmacy is required.
  • Knowledge of Medical Terminology and Coding
  • Experience with HEDIS requirements
  • Computer proficiency with MS Excel, Word and Access
  • Excellent customer service and interpersonal skills
  • Good verbal/written English communication skills. Spanish or other second language preferred.
  • Good organizational skills.
