Preferred: o Experience in geriatric pharmacy or long-term care setting o Excellent communication skills and interpersonal skills to collaborate effectively with other healthcare professionals. o Ability to handle a high stress environment Job Description: The Sterling Long-Term Care Pharmacist is a healthcare professional responsible for managing medications for residents of long-term care communities such as nursing homes, assisted living and rehabilitation centers, ensuring proper drug therapy by reviewing prescriptions, identifying potential drug interactions, and collaborating closely with other healthcare providers to optimize patient care within the facility. Core Competencies and Responsibilities:
- Accurately fill prescriptions, review medication orders for appropriateness, and check for potential drug interactions based on current medication list and allergy list.
- Collaborate with healthcare providers and nurses to discuss medication regimens, dosage adjustments, and potential side effects, providing expert advice on drug therapy management.
- Provide education to nursing staff about resident medications, including proper administration.
- Verify and update resident medication lists upon admission and transfer to ensure accurate medication management.
- Ensure adherence to all federal and state regulations as well as pharmacy policies, procedures, including proper storage, handling, and documentation of medications.
- Assist in training of staff pharmacists and pharmacy technicians.
- Supervise pharmacy technicians and support staff in day-to-day activities.
- Displays high level of professionalism (competence, integrity, respect, ethical).

- Displays excellent time management skills by identifying and efficiently working on the most important tasks first.

- Communicate with the pharmacy manager on a regular basis.
- Excellent computer operating skills. Displays proficiency in required software.
- Participate in the development of new programs and services for pharmacists which improve patient outcomes.
- Participate in continuous quality improvement and quality assurance program within the pharmacy.

Physical Requirements and Work Demands:
- Moderate physical activity. Requires handling of average-weighted objects up to 25 pounds or standing and/or walking for more than four (4) hours per day.
- Ability to work up to 40 hours per week and occasionally more.
- Participate in on-call rotation and respond to calls in a timely manner.
- Ability to maintain a flexible schedule including early morning, evening and weekend hours.
- In a typical day, the candidate can spend 2-4 hours on the telephone.
Working Environment:
- Work environment involves minimal exposure to physical risks, such as working with hazardous chemicals.
