About the Position: As a Health Home Care Manager, you will play a pivotal role in providing care coordination for clients dealing with chronic medical and behavioral health conditions. Your mission will be to assist clients in navigating the complex social service, community, and healthcare systems.

Key Responsibilities:

• Conduct comprehensive assessments and maintain detailed case notes.
• Facilitate enrollment in Benefit and Entitlement programs to support client needs.
• Ensure timely follow-up with newly referred clients.
• Provide Health Home Care Management services in community-based locations.
• Lead case record reviews and conferences to enhance client care.
• Coordinate with ongoing service providers, including mental health and medical specialists.
• Engage in case conferences with interdisciplinary teams to ensure cohesive care delivery.
• Perform fieldwork to connect with clients in the community.
• Maintain accurate data and case records as required by health home policies.
• Develop and update coordinated care plans in collaboration with clients and families.
• Effectively manage a caseload from initial assessment to discharge.
• Monitor client progress and document case notes meticulously.
• Participate in meetings to evaluate client care needs and discharge options.

Qualifications: A Bachelor's degree is required.

Position Details:

• Job Type: Full-time, Monday to Friday from 9 AM to 5 PM.
• Pay: $23.00 - $25.00 per hour.
