Job description We are seeking a compassionate and experienced Home Health Aide to join our team. In this role, you will provide high-quality care to patients in the comfort of their homes, supporting their physical, emotional, and personal needs. Key Responsibilities:
- Provide personal care and assistance with daily activities, including bathing, dressing, and grooming.
- Monitor and document vital signs (blood pressure, temperature, and pulse).
- Administer medication and provide catheter care as required.
- Assist with hospice care, offering emotional support and bereavement guidance.
- Provide care for patients with developmental disabilities and brain injuries (experience with brain injuries is required).
- Ensure a clean and safe living environment, following infection control protocols.
- Support with meal preparation and light housekeeping tasks.
- Develop and implement personalized care plans in collaboration with patients, families, and healthcare providers.
- Maintain detailed and accurate records of patient care and progress.
- Educate and support patients and families to promote independence and healthy living.
Qualifications: - High school diploma or equivalent required.
- Experience working with patients with brain injuries is required.
- Strong understanding of medical terminology and ability to follow medical instructions.
- Excellent communication and interpersonal skills.
- Ability to lift and safely transfer patients.
- Reliable transportation to and from patient homes.
- Current experience in patient care or a related field.
