This role is critical in optimizing discharge planning and ensuring smooth transitions for patients within the acute care setting. Key Responsibilities: - Collaborate with interdisciplinary teams including physicians and social workers to facilitate effective discharge planning. - Manage a caseload of 17-25 patients, ensuring timely referrals and coordination of care. - Function independently within the Med-Surg or Tele unit while demonstrating excellent time management and critical thinking skills. - Conduct patient rounds to assess ongoing needs and any barriers to discharge. - Utilize resources effectively to assist patients transitioning to home, community resources, or skilled nursing facilities. - Maintain up-to-date knowledge of healthcare regulations, policies, and standards. Qualifications: - Must have at least 1 year of inpatient case management experience; prior critical care experience is preferred. - Required certifications include BLS, as well as PRI and SCREEN certifications (which can be obtained online). - Familiarity with EMR systems, specifically Allscripts; knowledge of Utilization Review (InterQual, MCG) is preferred. - Strong communication and collaboration skills to work effectively within an interdisciplinary team. Working Conditions: - Full-time position with a schedule from Monday to Friday, 8:00 AM to 4:30 PM (with 7.5-hour shifts including a 1-hour lunch). - Occasional weekend and holiday shifts may be required with prior management approval. - Professional dress code: Business casual or colored scrubs (excluding Royal Blue or Maroon) is required. Lab coats will be provided. - Comprehensive orientation, including EMR training, followed by 1-2 weeks of training with a preceptor.
