Job DetailsJob Location ORANGE, CT Position Type Full Time Salary Range $100000.00 - $150000.00 Salary/year Travel Percentage Road Warrior Description Constellation Hospice is seeking an energetic, dynamic individual to work as part of our hospice sales team as a Hospital Liaison.The Liaison will be responsible for expanding business opportunities in Hospitals, Skilled Nursing Facilities, Assisted Living Facilities and Physician Offices.This person will manage the clinical care coordination and work with the Clinical Directors and Intake as needed on the continued development of post-acute care facilities in the service area. The Liaison is directly involved in managing the multiple clinical elements that comprise a person's successful transition from one level of care to another. Assess clinical status and eligibility for the identified benefit, discuss care goals, and recommend appropriate service selection. This candidate should demonstrate exemplary communication, organizational skills, and knowledge of clinical eligibility with clinical assessment skills to identify the proper disposition of care needs. - Responsible for generating referrals for our hospice program by building relationships with social workers, case managers, wellness directors, physicians, and key decision-makers.
- Conducting market analysis, developing a sales strategy, quarterly goals, and plans, conducting sales calls, and evaluating results and effectiveness of sales activity.
- Assesses clinical status, and eligibility for the identified benefit, to determine the appropriateness for Hospice admission based on Hospice Criteria.,
- Discusses clinical goals of care and makes recommendations for appropriate service selection.
- Performs full clinical assessment and handles GIP admissions and visits.
- Clinical focus on ensuring the patient's eligibility, required consent forms, correct coding and billing documentation.
- Develops and manages relationships with referral partners within the assigned post-acute accounts and in the community.
- Identification and prioritization of potential referral partners, including decision-makers within each account.
- Commutable to all geographic territories as needed for assessments, training, orientation and referral source visits

- Licensed RN in the state of Connecticut.
- Minimum of 1-3 years of clinical experience in Hospice and/or Palliative Care.
- Ability to assess patient clinical status and discuss care options for the state of illness.
- Previous hospice sales experience preferred.
- Bachelor's degree preferred
- Excellent planning, organization and presentation skills are critical.
- Proven ability to develop and implement a sales and marketing plan.
- Excellent oral/written, communication and interpersonal skills.
- Computer Skills and basic knowledge of iOS operating system.
- This position requires an individual who is highly motivated, results-driven, and able to develop and build strong, lasting relationships.
- The ideal candidate will have established healthcare contacts and be able to readily network in the community.
