About the Opportunity: One of our esteemed clients, a leading Certified Home Health Agency (CHHA), is seeking dedicated Registered Nurses to join their team as Coordinators of Care. These field-based roles cover Manhattan, Queens, and Brooklyn, with a car and valid driver's license required for Queens and Brooklyn regions. In this role, you will be the cornerstone of your patients' home care journey, taking primary responsibility for coordinating, implementing, and continuously evaluating their care needs. You'll collaborate with families and caregivers, ensuring that each care plan is effectively executed and tailored to the individual. Your strong communication, documentation, and organizational skills will allow you to thrive in this role, all while enjoying the autonomy and respect you deserve. Field Nurse Key Responsibilities:
- Develop, implement, and evaluate individualized care plans for home care patients.
- Review patient insurance information and coordinate care activities.
- Share critical findings with patients' families and interdisciplinary caregivers.
- Manage your time efficiently to ensure optimal care delivery.
Field Nurse Qualifications:
- Associate's Degree in Nursing (required); Bachelor's Degree in Nursing (strongly preferred).
- Minimum of 1 year of acute, medical-surgical nursing experience (required).
- Prior experience in CHHA or LTHHCP settings (preferred).
- Proficiency in Microsoft Office and clinical software systems (preferred).
- Valid driver's license and reliable transportation (required for Queens and Brooklyn).
- Bilingual fluency in Spanish, Cantonese, or Mandarin (a plus).
