We are seeking a Community Health Registered Nurse (CHRN) who is passionate about providing high quality primary healthcare to high risk populations, including those in our Healthcare for the Homeless program. Do you know case/care management principles, do you find joy in working with patients with various backgrounds, and have an understanding of social drivers of health? This Monday-Friday role may be for you.
About the Role:
As the CHRN you'll use your training, critical thinking, and kindness as you consider patient cases, identify solutions or suggest alternate approaches to patient problems. Much of your interaction with patients will be over the phone, although you are open to meeting patients where they are and engaging patients in a variety of settings. You'll be part of a clinical team that collaborates to design and implement individualized patient-centered care. Essential duties include:
Initiate care plans, and manage/coordinate care; identify and address barriers affecting patients' ability to adhere to healthcare plans
Build trusting relationships with patients to facilitate access to health care services and meet health needs
Assist patients and families with navigating Neighborhood's services as well as outside community resources, programs or services
Attend community events as an RN to provide health education
Participate in some evening street/shelter outreach, getting to know potential patients, assessing needs, and encouraging them to establish care
You will be primarily based at our Mattina location, 300 Niagara Street in Buffalo, and will also travel to other Neighborhood sites on a regular basis.

Education and Experience to be a Community Health Registered Nurse:
Degree from accredited two year nursing school
Current NYS Professional Registered Nurse license
2+ years clinical nursing experience
CPR certification
Knowledge of case/care management principles
Attention to detail, organizational skills, project and time management skills, and ability to follow through

Resiliency: you see opportunities to innovate and find solutions when challenges arise
Teamwork: you are open to others' unique perspectives, and will collaborate to meet shared goals
Knowledge of Microsoft Office Suite, Adobe, and database management

Compensation starts at $36.50 per hour.

RN Tuition Reimbursement Potential: Our status as a Federally Qualified Health Center (FQHC) means registered nurses have an opportunity for tuition reimbursement up to 60% repayment of qualified student loans directly related to a nursing degree. RNs who are granted reimbursement are required to fulfill a two year, full time commitment to maintain eligibility. The reimbursement program requires an application and is not guaranteed.
