Job Description
Provides screening, orientation, assessment, and counseling services to patients. Performs individual and group counseling to patients and families. Coordinates intakes and contributes to the treatment process.
Job Responsibilities
Application / Assessment / Intake• Answers calls from individuals seeking information and assistance related to chemical dependency issues and related problems. Offers basic information regarding chemical dependency and service availability, provides immediate crisis intervention if necessary, completes the pre-screening form, and establishes intake appointments as requested.• Interviews applicants requesting admission, completes designated counseling sections of the Integrated Assessment Form, and provides an overview of treatment goals. Explores treatment options and explains levels of service.• Identifies problem areas for treatment, lists them in the Clinical Formulation/Integrated Summary, notes applicant's strengths, and completes Axis I, II, IV, and V. Provides relevant medical information to the healthcare provider and makes treatment recommendations based on needs and eligibility.• Provides referrals for those not accepted into treatment.• Recognizes signs of co-occurring psychiatric disorders and recommends psychiatric evaluation when necessary.Treatment Planning• Reviews assessment information and presents it at case conferences.• Develops an Initial Treatment Plan with the interdisciplinary team, outlining problems, goals, activities, responsibilities, and timelines.• Completes a Bio-psychosocial Addendum for a comprehensive understanding of the patient.• Monitors progress, reviews and updates the Treatment Plan, and manages discharge planning and aftercare.• Conducts HIV Risk Assessments semi-annually and follows up accordingly.Case Management• Oversees treatment, coordinates referrals, and resolves patient issues.• Manages financial assessments, billing, and insurance communications.• Monitors toxicology results, advocates for patients, and supports family involvement.• Ensures appointment adherence, schedules follow-ups, and assists with accessing services.• Manages patient movement between care levels and provides orientation and support.Counseling• Establishes rapport and provides orientation, individual counseling, crisis intervention, and group sessions.• Engages family members when appropriate, offers gender-specific treatment, and adapts techniques for co-occurring disorders.Patient Education• Identifies educational needs, develops materials, and conducts seminars.• Leads outreach activities and assists patients in resource navigation.Environment and Emergency Situations• Monitors patient behavior, enforces policies, reports emergencies, and completes incident reports.Documentation• Records significant events, activities, and referrals.• Completes notes timely and accurately, obtains necessary consents, and maintains compliance with policies.• Prepares reports for third-party payers and completes required forms.Performance Improvement• Participates in reviews, mentoring, and quality projects.Professional Development• Engages in training, professional organizations, and community activities.• Attends seminars and conferences to enhance skills and credentials.• Participates in staff meetings.Other duties as assigned, adhering to ethical principles and organizational policies.Job Qualifications
- Master's Degree in a related field required.
- Licensed Professional LMSW/LCSW/LMHC.
- CASAC-T certification within one year of hire; CASAC certification within three years.
- Minimum of two years full-time experience in Behavioral Health.
