Position Summary:

Under the direct supervision of the PACE Senior Program Director, the PACE Center Manager is responsible for a wide range of disciplines and services provided through the PACE Center. The PACE Center Manager is responsible for all administrative oversight related to the direct operation of the Center which includes a day center and a PACE clinic setting. The Center Manager coordinates overall operations of the PACE Center and serves as the liaison between direct care staff and administration.

Essential Functions of the Job:

  • Hire, supervise, lead and coordinate the PACE Inter Disciplinary Team (IDT) Moderators in an effort to ensure utmost regulatory compliance with PACE regulations.
  • Ensure the recruiting, selecting, orienting, and training of employees; maintains a safe, secure, and legal work environment; developing personal growth opportunities.
  • Communicate job expectations with employees to achieve results; manages employee performance through coaching, counseling, and disciplining and conducts all performance evaluations for the nursing team; prepare work schedules and ensure employees have necessary resources to succeed.
  • Coordinate all center activities including care planning and implementation, preparation for interdisciplinary conference and daily meetings, family conferences, and initial and periodic assessments.
  • In collaboration with PACE Day Center Assistant Manager, coordinate services and activities offered in the day center and ensure we are meeting state regulations and appropriate staffing ratios.
  • In collaboration with Senior Compliance Analyst ensures the -execution, maintenance, and coordination of subcontracted services.
  • Serve as a representative of PACE to the community.
  • In collaboration with PACE Director manage and work within the PACE Center budget and regularly communicate with the finance department on the Center's operations and needs.
  • Streamlines the facilitation of the interdisciplinary team (IDT) meetings, as well as reoccurring scheduled Intake and Assessment meetings to review participant cases on a regular basis.
  • Oversee the quality of care and safety of PACE participants, both at the PACE Center and in the community (i.e. at home, in nursing care facility, during hospitalizations, etc.).
  • Serves as a liaison between disciplines, administration, and direct staff.
  • Work closely with the primary care physician staff, technical managers and leaders to develop and implement participants' individualized care plans.
  • Coordinates with marketing department promote PACE services in the community and hold “friendly visits” for new candidates at the PACE center.
  • Ensures maintenance and coordination of subcontracted services.
  • Coordinates all activities related to new participants.
  • Coordinate 24-hour care delivery.
  • Manage and work within the PACE Center budget and regularly communicate with the finance department on the Center's operations and needs.
  • Proactively engage with PACE Administration to further program development, including participation in working committees and project development/implementation.
  • Oversee overall documentation cohesiveness and accuracy in each participant's medical record.
  • Be proficient in and become a super user of PACE Electronic Health Record (EHR) system and in collaboration with the IT Department, ensure the initial staff training and on-going proficiency on and appropriate use of PACE EHR.
  • Review monthly operation expenses to assure budgetary compliance and to identify any budget variations as appropriate.
  • Participate in budget planning process and review.
  • Responsible to ensure compliance in all areas of program operations and to prepare the necessary Corrective Action Plan (CAP) as needed in a timely
  • process. (CMS; DHCS, Clinic; Dept. Of Education; DHCS, Office of Long Term Care)
  • Responsible to investigate, monitor, and track all PACE provision of service lifecycle, participant grievances and incidents to ensure prompt resolution to their issues.

Additional Duties and Responsibilities:

  • Provide oversight for and coordinate day center activities and services
  • Performs other duties as assigned

Job Requirements:

Education Required:

  • Bachelor's Degree in health related or social service field

Education Preferred:

  • Master's Degree in health, business or social service field

Certifications/Licenses Required:

  • CA valid driver's license and reliable transportation
  • Proof of current vehicle insurance (if applicable)
  • CPR and First Aid certification

Experience Required:

  • Minimum of 3-5 years management or leadership experience.
  • Experience in project management and group facilitation

Technical Knowledge and Skills Required to Perform the Job:

  • Must possess the skills necessary to provide care for geriatric patients, must thrive in team environment, possess good organizational and supervisory skills, and have the ability to effectively handle difficult and unusual interpersonal situations. Meet a standardized set of competencies for the specific position description established by SYH PACE and approved by CMS.

Working Conditions and Physical Requirements:

  • Physical clearance for reaching, bending, stooping, crouching, kneeling and grasping; have full range of motion; ability to lift 50 lbs.; need to lift and transfer participants; T.B. clearance. May be required to work evenings, holidays and weekends
