This is accomplished by connecting participants to necessary support services, coordinating with other care providers, gathering resources, and assisting with applications and renewals when necessary. This position also assists adults with serious mental illness in pursuing the outcome of improved or maintained mental health functioning. The Community Support Program (CSP) provides community-based programming, service, and resources toward the empowerment of community living with chemical and mental health barriers. It is a drop-in based studio with an emphasis on Art. This position will also work in coordination with our CSP program to provide case management services to members of that program. This position will work with CSP staff in assisting members to pursue the outcome of improved or maintained mental health functioning. This individual will also collaborate with teammates to ensure service delivery is equitable and accessible. On a rotational basis or as needed with the rest of the CSP staff, the case manager will conduct intake of new members, assist members with their own self-directed recovery plan, and provide coverage at the CSP. PRIMARY RESPONSIBILITIES FOR TCMProvide direct client services:
  • Build meaningful and purposeful client relationships with appropriate boundaries.
  • Able to inform and educate others on eligibility for programs and services.
  • Make appropriate referrals internally and externally.
  • Complete 3 or more creative attempts per client if needed.
  • Use creative strategies to engage in goal planning.
  • Engage and build rapport with clients.
  • Assess eligibility for program and services and connect individuals to those desired and necessary services.
  • Assure that services provided, as developed by the client and case manager, are reflected in the Individual Community Support Plan. If clients' goals change substantially, the case plan should be updated to reflect this.
  • Make appropriate referrals, monitor progress, and ensure follow-up.
  • Utilize creative approaches to meet in person, regularly with clients and address their needs.
  • Immediately consult about any safety related concerns (for client and staff), mandated reporting concerns, and complaints from the client.
  • Meet with all clients on caseload, at least monthly and complete a billable case note for each client.
  • Assist in locating housing for individuals on the caseload as needed.
  • Support caseload in obtaining resources for moving into housing as needed.
  • Actively address concerns which may arise, related to their housing stability.
  • Ability to successfully collaborate/coordinate care with other providers to keep clients housed or moved to appropriate housing resources.
PRIMARY RESPONSIBILITIES FOR CSP
  • Complete CSP intakes with clients as needed. Review the assessment of needs, establish a case plan designed to meet the immediate service needs and set up on-going services needs in the community.
  • Coordinate physical and behavioral health management using the concept of Whole Person Care while addressing both client-center goals and health related social needs to efficiently improve overall health of the client.
  • Coordinate services and supports necessary to assist each client as they work toward their goal of recovery.
  • Assure that all changes in client status or needs are communicated through documentation.
  • Work with other CSP staff to communicate goals, concerns, and techniques to effectively work with individuals.
  • Assist in coordinating care with clients' medical providers i.e., PCP, dentist, specialty providers and preventative care.
  • Provide collaboration and education to significant others and family members who are interested in assisting the client in their recovery process to the extent requested by the client.
  • Advocate on behalf of each client to assure the protection of client rights and privacy, as well as assure that clients understand the complaint and grievance procedures.
  • Act in a manner to assure client confidentiality and privacy that complies with MHR policies and procedures/HIPAA/MN Medical Records Act.
  • Connect client to necessary community agencies to ensure closure in a timely manner.
QualificationsPOSITION REQUIREMENTSEducation and ExperienceRequired
  • Bachelor's Degree in a Human Services related field.
  • 2,000+ hours of supervised experience working with adults with mental illness.
  • Ability to provide Rule 79 Adult Mental Health Targeted Case Management per DHS guidelines.
  • One or more years of experience working with single adults experiencing homelessness or similar experience.
  • Lived experience will be valued and considered as part of the experience requirement.
Desired
  • Two years case management experience of which at least one year is experience in community based mental health with adults recovering from mental illness or combination of equivalent experience and training.
  • Previous work experience in a behavioral health setting.
  • Work experience in a culturally diverse environment.
  • Experience with programs serving individuals with dual diagnoses.
  • Experience with or interest in working with children's mental health.
