: Job Details :

I. SPECIFIC RESPONSIBILITIES - Coordinate and schedule screenings, intakes and admissions of potential residents for placement in Crisis Respite and Community residence programs (CR & CCR). - Facilitate and schedule Service Plan reviews (initial, quarterly, annual) with multi-disciplinary team of staff, service providers, child and family in the development/update of goals - Administer CANS assessment to all residents at allocated intervals. - Completion of Biopsychosocial Assessment at time of admission and update annually. - Participate/facilitate parent training groups/meetings and family engagement program activities monthly. - Arrange/ Coordinate sibling and family visits/contacts in the program, community and home - Provide crisis de-escalation with children and meet with them individually and in groups to work on specific service plan/ treatment goals. - Participate in bi-weekly SPOA Out-of-Home Placement Committee meetings as assigned - Work in conjunction with the CR Team and ...Social Worker, Worker, Retail, Program
