The Case Manager assists the physicians and staff to determine the appropriate level of care and post-acute care needs.This position reports to the Director, Case Management and Social Work and operates within established organizational and departmental policies and procedures.Qualifications
- Graduate of an approved, accredited school of professional nursing required; BSN preferred
- Licensure in the Commonwealth of Massachusetts as Registered Nurse required
- Case Management certification or active pursuit of certification preferred
- One to two years of related clinical, continuing care, or case management experience preferred
- Excellent written and oral communication skills required
- Excellent computer skills required including MS Office, EPIC and 4Next required
- Willingness to be flexible, resourceful, and creative in problem solving required; high degree of delegation and prioritization skills required
- Excellent interpersonal and relationship building skills to effectively collaborate with multiple stakeholders to obtain staff and physician involvement and accountability in achieving specific patient outcomes required
- Demonstrated ability to orchestrate many activities and manage multiple priorities required
