Job Description :
Position: In-Home Preventative Health Risk Assessment ProviderJob Summary Conduct no-cost annual preventative health risk assessments for members of Medicare, Medicaid, and Affordable Care Act plans, performed in the member's home per health plan expectations.Work Hours
  • Flexible scheduling: Daytime and evening availability
  • Each appointment: 60–75 minutes
Compensation
  • Market rate per completed assessment
  • Mileage reimbursed after first 30 miles driven per day (at current IRS rate)
Responsibilities
  • Complete self-paced and instructor-led training within designated timeframe
  • Maintain a minimum of 30 appointment slots weekly
  • Ensure member satisfaction and adherence to quality standards
  • Document and submit chart notes within 24 hours post-visit
  • Respond to charting queries within 48 hours
  • Collaborate closely with clinical leadership
  • Participate in periodic training and team meetings
  • Comfortably enter member homes for scheduled visits
  • Lift up to 20 lbs as needed
  • Bend or kneel during assessments as required

  • Strong computer skills; proficient in electronic medical records
  • Knowledge of CMS and ACA risk adjustment models
  • Accurate and thorough documentation
  • Effective verbal and written communication with members and staff
  • Confidentiality and HIPAA compliance
  • Critical decision-making aligned with policies
  • Adaptability to changing responsibilities
  • Positive and collaborative attitude
  • Proactive resolution of member/provider/plan concerns
QualificationsEducation & Experience
  • Master's or Doctorate from accredited NP program (FNP, AGNP, ANP) or Master's from accredited PA program
  • Home health experience within the last 5 years required
  • 1 year of in-home assessment experience preferred
Licensure & Certifications
  • Active RN and NP or PA license in state of practice
  • Board certification from ANCC, AANP, or NCCPA
  • Active Medicaid ID and Medicare PTAN
  • Current BLS certification
  • Unencumbered driver's license
  • Valid car insurance in state of scheduled appointments
