Position Overview

As a Physical Therapist (PT), you'll provide patient-centered care in a Home Care setting. You'll play a crucial role in helping clients regain mobility, improve independence, and reach their therapeutic goals.

Key Responsibilities

  • Evaluate and treat patients recovering from injury, illness, or surgery.
  • Develop customized care plans using evidence-based therapy techniques.
  • Track patient progress, adjust treatment goals, and document results.
  • Recommend and train patients and families on assistive devices and therapy techniques.
  • Collaborate with a multidisciplinary team to ensure quality care delivery.
  • MDS experience is a plus

What You Need to Join Us

  • Bachelor's degree (or higher) in Physical Therapy.
  • Valid New York State Physical Therapy License.
  • Strong knowledge of rehabilitation methods and medical terminology.
  • Experience in acute care, pediatrics, orthopedics, or home care is a plus.
  • A passion for helping others and a team-first mindset.
