We are seeking a Transitional Healthcare Liaison for a full-time position at the Indianapolis Parole District in Indianapolis, Indiana. The Transitional Healthcare Liaison will be responsible for engaging with recent parolees post-release to assist with and support the continuum of care and provide resources and community-based services. The Transitional Healthcare Liaison will provide education and support to the released offenders to enable a successful transition into their community, connect them to community resources, and assist in obtaining services.
