- Conduct comprehensive initial assessments, with a focus on both substance use and mental health needs
- Collaborate with the clinical team to develop and support individualized treatment plans
Ongoing Monitoring & Clinical Support
- Monitor client progress and communicate updates to clinical staff
- Evaluate the effectiveness of care plans and adjust interventions as needed
- Ensure medications are administered accurately and in accordance with protocols
Documentation & Records
- Maintain detailed, timely, and organized documentation in the electronic medical record system
- Support quality care through accurate charting and compliance
Client Education & Advocacy
- Provide clear and supportive education to clients and families about mental health, substance use, and physical well-being
- Foster a safe, nonjudgmental, and empowering atmosphere for healing
Team Collaboration
- Partner closely with therapists, case managers, and leadership to deliver wraparound support
- Participate in interdisciplinary discussions and contribute your clinical expertise
Requirements
- A licensed Registered Nurse in the state of New York (active and unrestricted)
- Naturally empathetic, with strong communication and interpersonal skills
- Detail-oriented and organized, especially when it comes to documentation and medication protocols
- Comfortable working independently but collaborative by nature
- Passionate about helping others
- Experience in addiction or mental health care is a plus, but not required (we'll train you!)
