The Registered Professional Nurse assesses patient needs, develops a plan of care in collaboration with the interdisciplinary team, provides direct care, evaluates patient progress, and coordinates care delivery throughout the patient's episode. The RN is accountable for patient family education to assist in understanding self-care beyond hospitalization, including lifestyle changes to support health and healing. The Registered Nurse is expected to promote comfort and provide emotional support in a healing environment. The RN applies nursing knowledge and skill in the delivery of care. They assess patient/family needs through active listening, social perceptiveness, and analyzing data or information obtained through direct observation, assessment, technology, or health team communication. They create a developmentally and age-appropriate, culturally relevant, individualized plan of care to address the nature and scope of the health problems identified. In collaboration with the patient, family, physician, and healthcare team members, the RN sets goals and monitors the patient's progress. The RN adjusts the plan based on patient response or changes in condition or reprioritization of needs. They promote, advocate for, and protect the health, safety, and rights of the patient. The RN encourages and builds mutual trust, respect, and cooperation among team members to accomplish the best possible patient outcomes. They teach others as appropriate and delegate tasks and interventions consistent with the individual's education, training, and scope of practice. The RN understands the personal responsibility to safeguard public safety through compliance with infection prevention strategies. They motivate the patient/family by teaching them what is needed to continue care/healing at home. The RN assesses the patient/family learning styles and structures sessions to maximize understanding. They seek teachable moments during care delivery to emphasize pertinent information. The RN asks questions to evaluate retention of key points in the self-care process. They provide tools and information to assist the patient's family after discharge. The RN documents the education provided and the patient/family's response to the education. They systematically evaluate the quality and effectiveness of nursing practice and use the results of quality improvement activities to initiate changes in individual nursing practice and care delivery. The RN engages in self-evaluation of practice, identifying strengths and areas in which professional development would be beneficial. They take accountability to increase knowledge and enhance skill. The RN uses logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems. Nursing practice reflects the importance of making a meaningful connection with the patient/family.
