You'll collaborate with clinical staff to design and implement meaningful and individualized patient-centered care. Responsibilities include:* Work with patients to maximize their health capabilities* Perform triage and manage the triage flow* Administer medication and provide treatment prescribed by provider* Perform immunizations and injections* Continuous collection of health data about the health status of patients* Assist with patient flow for the providerYou will travel to all 6 of our sites in Western New York as needed.What it's Like to Work at Neighborhood:The top three words employees say describe the work environment are: teamwork, supportive, kind.
