The Regional Home Health Quality and Patient Experience Coordinator reports to the Home Health Quality and Patient Experience Zone Manager and is responsible for facilitating overall agency quality improvement and outcome performance. 50% travel required within the state of Massachusetts. Responsibilities Performs at least one on-site survey readiness assessment annually using designated protocols and tools as requested Partners with agency leadership in developing and oversees implementation and monitoring of effective actions to mitigate quality related findings identified in the survey readiness assessments as applicable.
