Job Description:We are seeking a compassionate and skilled Registered Nurse to join our Home Health Hospice team. In this role, you will provide in-home nursing care, support patients with chronic or terminal illnesses, and help them manage their care with dignity and comfort.
Key Responsibilities:
Provide 5 patient visits per day in a 40–80 mile territory
Deliver skilled nursing care, education, and support to patients and families
Develop and update individualized care plans in collaboration with the care team
Assess and respond to patient needs, including physical, emotional, and spiritual
Work one weekend per month and participate in an on-call rotation (5–6 times/month)
Document care provided and changes in patient condition in EMR
