BHcare is seeking aCommunity Case Managerto provide compassionate, trauma-informed care to survivors at high risk, helping them reclaim safety, stability, and self-determination.This position is part-time, 16 hours per week. Monday and Tuesday from 9am-5pm, with the potential of flexibility in days worked.Who We AreBHcare is a Certified Community Behavioral Health Clinic offering integrated care for individuals and families navigating mental health, substance use, and domestic violence. Apply today!QualificationsMINIMUM QUALIFICATIONS:
  • Bachelor's Degree required
  • Bi-lingual (English/Spanish) preferred
  • Two years Advocacy and/or direct service experience working with survivors of domestic violence in a community-based setting preferred
  • Experience assisting traditionally underserved populations with a developed understanding of issues of diversity essential
  • Crisis intervention and counseling skills
  • Knowledge of community resources
  • Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
  • Must be able to obtain and maintain the domestic violence counselor certification per CGS 52-146K before providing direct service to clients, followed with six-hour recertification yearly
  • Must have and maintain a valid driver's license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization's auto insurance plan.
