San Diego's St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul's PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.

St. Paul's PACE is seeking a Primary Care Physician that can:

  • Coordinate with the interdisciplinary team to develop a comprehensive care plan for each participant.
  • Integrate the primary care treatment plan into the overall plan of care developed by the interdisciplinary team. Interacts with team members to meet emergent and acute need of participants.
  • Create a welcoming environment for all St. Paul's PACE Participants and develop inspiring, unforgettable relationships!

Key Responsibilities:

As a Primary Care Physician at St. Paul's PACE, you will:

  • Deliver high-quality, comprehensive primary care to frail older adults in coordination with an interdisciplinary team
  • Perform initial, semi-annual, and ongoing assessments to develop individualized care plans
  • Integrate the primary care treatment plan into the broader plan of care developed by the interdisciplinary team
  • Evaluate and manage chronic conditions and acute illnesses
  • Provide preventive care, including immunizations, screenings, and health maintenance
  • Participate in care planning and discharge decisions for hospitalizations or long-term placement
  • Communicate effectively with nursing staff, specialists, and other providers to ensure seamless, coordinated care
  • Make home, assisted living, and nursing facility visits as needed
  • Provide ongoing clinical support and training to staff
  • Participate in team meetings, peer review, quality improvement, and compliance efforts
  • Contribute to clinical policy development and program evaluation under the guidance of the Medical Director

Education:MD/DO with interest in geriatrics. Board certified or Board eligible (if newly graduated from residency) in Internal Medicine, Family Practice, or Geriatric Medicine desired. Current state license and DEA registration required. Meets requirements of 42 CFR Part 410.20.

Experience:A minimum of one year experience working with a frail or elderly population required. Shall have either training or related experience in the assigned job.
