Job Requirements
- Licensure & Certification:
- Current State Pharmacist license (active at time of start)
- Successful completion of educational requirements for state licensure
- BLS (if required by state)
- Experience:
- Minimum 2 years recent hospital pharmacy experience
- EPIC EMR proficiency
- Specialized Clinical Knowledge:
- Medication reconciliation for discharge
- Antimicrobial stewardship (e.g., interpreting cultures, managing UTI/pneumonia/SSTI)
- Pharmacokinetic dosing/monitoring
- Anticoagulation dosing/monitoring
