Florida Health Care Plans, Inc. is seeking a full-time LCSW or an LMHC with 3 years' experience to join our Behavioral Health Team in Edgewater. Office hours are Monday-Friday, 8-5, outpatient only. The ideal candidate will have experience in dual diagnosis of substance abuse and mental illness, group, individual, family therapy & case management. Must be comfortable with children, adolescents, and adults. Duties and Responsibilities Case list will consist primarily of substance abuse patients and dually diagnosed Facilitating groups within the department focused on substance abuse, relapse prevention and mental health stability. Evaluation, assessment, and treatment of dually diagnosed patients. Promotes positive patient relations Completes required documentation in a timely and legible manner consistent with appropriate documentation standards. Coordinates with clinical staff (crisis team, nursing, psychiatry, therapy) in assisting patients and their families in understanding, accepting, and following medical recommendations Performs independently and manages other duties as assigned.
