DescriptionThe Outcomes Management RN assesses the level of compliance with defined evidence-based practices through direct observation and chart review processes of care in accordance with benchmarking organizations' methodologies. This position works collaborates with direct care providers to ensure patients are receiving appropriate care during their hospitalization.Core Competencies
- Conducts concurrent chart reviews to identify potential quality of care issues and works corroboratively with direct care givers when possible.
- Works collaboratively with the Health Information Management team and staff to promote cooperation and understanding of the data collection process
- Demonstrates an expert level of knowledge in assessing care according to specific guidelines thereby promoting consistency in methodology and validity of the resulting data.
- Understands data storage and data sharing methods. Investigates data to find patterns and trends. Converts data into usable information that is easy to understand.
- Assists in developing and maintaining current policies and procedures, which will promote consistency in data collection, analysis and reporting systems.
- Audits reports from benchmarking organizations to ensure validity and takes immediate action to resolve discrepancies.
- Collaborates with and assists the Clinical Quality and Accreditation Research Analyst in the aggregation and submission of data according to standards and within establishes timelines.
- Assesses potential or known future changes in data collection, submission and reporting to benchmarking organizations and collaborates with appropriate internal management and staff to develop immediate strategies for compliance.
- Assists in the development of communication tools which promote understanding of the benchmark reports and how the data applies to each discipline's practice; thereby promoting ownership of the outcomes and buy-in to seek improvement.
- Participates in teams when requested with clear understanding of how to participate in an effective and efficient meeting.
- Demonstrates effective interactions with physicians and other clinicians, managers and staff at all levels within the Medical Center.

- Participates in the ongoing evaluation of the structure, processes and outcomes of the data collection and benchmarking systems. Works with HIM management and the coding team in developing and maintaining current materials to prompt clinicians to accurately document (example: query forms). Actively manages the ongoing evaluation of the CDI program and provides creative, constructive, and realistic recommendations for change.
