Therapy/Rehabilitation/PT Home Health: Duration:13 Weeks Start:06/16/2025 Shift: Day 5x8-Hour (08:00 - 16:30) Description: Physical Therapist in Home Health provides rehabilitative care to patients in their homes to improve mobility, strength, and independence. They assess patient needs, develop personalized treatment plans, and educate patients and caregivers on exercises and safety. The role involves traveling to patients' homes and coordinating with the healthcare team. Strong communication and self-management skills are essential. Home Health PT - Apple Valley and Surrounding Areas (Apple Valley, Lucerne, Victorville, Adelanto, Hesperia, Oak Hills, Phelan, Pinion Hills, Barstow and Helendale) - Ratios: 1:1 (*May visit up to 6 patients per day; 30 productivity points standard) - Years of Experience REQ: 1 YR of Skilled Home Health Experience - First-Timers Accepted: No - Weekend REQ: 1 Weekend Shift Per Contract - Certs REQ: BCLS - Is on-call REQ:No - Will clinician float within scope to meet facility needs:No - Open to accommodating block schedule:No - Locals Accepted: Yes; Considered local within 50 mile radius, will accept locals at the same rate - Pending License Accepted: Yes - RTO Restrictions: Confirm with hiring manager - Guaranteed Hours: Guaranteed Work Week (per contract)! Client agrees to pay all weekly hours; travelers will not be called off shifts Special Requirements: - Traveler is expected to have their own transportation. - System does a good job grouping clients minimizing drive and windshield time.
