Job Summary: The Patient Care Coordinator is accountable and responsible for managing the clinical care of course of treatment of a number of patients. Coordination of care is accomplished through collaboration with physicians, associate staff, and other health care providers to ensure that patient outcomes are achieved within the projected course of treatment using the nursing process. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. Performs RN duties as staffing needs require. Performs Medication Reconciliation process on admission, discharge and as needed. Performs required documentation/calls to physician on communication of lab results, imaging results, change in status and any resident needs and charts reviews from iRounds. Performs unit specific duties to include, med room checks, medication dispensing system discrepancy, code cart checks, temp log checks for wipe warmer and med room refrigerator; delegate as needed.Performance Expectations:

  • Demonstrates the ability to Round with the physicians, knowing important information on each patient they are rounding on.
  • Demonstrates the technical aptitude for working and troubleshooting equipment used including but not limited to traction, bladder scanner, wound vacuum systems, continuous passive motion machines, and specialty beds.
  • Demonstrates the aptitude for safely positioning and assisting patients using a variety of devices such as but not limited to abduction pillows, splints, slings, braces, neck collars, walkers, crutches and immobilizers.
  • Implements a systematic and comprehensive plan of care to provide individualized care, educates patient and family about diagnosis, treatments or prescribed regimen and medications in accordance with patient and family readiness and ability to learn.
  • Demonstrates knowledge and giving report on each patient during the Care Team conference and the QAPI meetings.
  • Demonstrates knowledge of patient care process if taking patients for the day, knowing how to administer medications, IV's, respiratory treatments, dressing changes, etc.
  • Demonstrate pressure ulcer documentation knowledge in the electronic medical record as scheduled per Director.
  • Demonstrate pressure ulcer documentation knowledge in the electronic medical record as scheduled per Director of Nursing as applies weekly.
  • Performs iRounds on residents and documents daily in PressGaney; during dayshift hours.
  • Completes hand hygiene observations; daily during day shift hours.
Qualifications:Education/Skills Graduate of a recognized non-online RN program required. Work Experience: Two years as a staff RN in a long term care or hospital setting required, with six months being at Forrest General Hospital preferred. Certification/Licensure-DUE UPON HIRE
  • Licensed RN able to practice within the State of MS
