Job Type Full-timeDescriptionStavros is actively seeking to fulfill an opening within the One Care Department to help coordinate needed services for people with disabilities who live at home. The primary responsibilities would include person centered care planning, coordination of services and supports that prevents people from going into a nursing facility. This position will also serve as an advocate for consumers needing services to meet their needs. As one of the Oldest Centers for Independent Living in the Country Advocating for Individuals with Disabilities and the Deaf to maintain their independence, Stavros is a Non-Profit Organization that prides itself in its culture by embracing applicants with diverse backgrounds. Requirements•Conduct assessments; by identifying the needs and recommending community services and resources for members with disabilities •Connect consumer to social services and community resources•Participate on the consumers care team•Perform outreach•Advocate for the member•Coordinate care services•Initial and quarterly home visits•Remain current with community resources•Must have knowledge of community resources and services available for people with disabilities.•Bachelor's degree in Social Work or Human Services or at least 2 years working in a human service field•Knowledge of home and community-based service systems and how to access and arrange for services•Excellent computer skills, knowledge of MS Office and database navigation•Excellent communication and writing skills to ensure respectful delivery of services•Excellent time management and organizational skills•Must have valid MA driver's license and proof of insurance
