Resident Services Coordinator, R3 Initiative
Position Summary
HSL provides enhanced housing with services to seniors living in its three affordable housing locations, and at partner housing sites, with the goal of improving quality of life and supporting independent living. For this role, you will work at 61 Park St in Brookline, Massachusetts, a 100-unit Brookline Housing Authority elderly and disabled property. Our resident service model is proactive, with staff actively reaching out to residents to engage them in their health and wellness, identify needs/risk, and provide individualized case management and support.
The ideal candidate is a social worker or case manager with experience in senior services, mental health, or healthcare navigation.
The Resident Services Coordinator (RSC) leads resident engagement in wellness assessments, health education, connecting residents to services, and managing casework, with support from the Nurse Care Manager and Programming Coordinator.
The RSC ensures excellent communication and coordination with partners, including BHA staff, emergency responders, payers, and local providers, to support residents' independence and safety.

Position Responsibilities
- Conduct wellness assessments and develop wellness plans covering physical, social, cognitive, spiritual, and mental health
- Coordinate with healthcare providers and hospitals
- Partner with housing staff to tailor resident programming
- Educate staff on identifying and communicating resident health changes
- Complete and maintain resident files with health and emergency information
- Follow up with at-risk residents to ensure adherence to wellness activities
- Develop relationships with payers and facilitate communication between housing and providers
- Promote self-care and coordinate with care providers to support resident goals
- Make referrals to local service providers (e.g., Visiting Nurses, PACE, Memory Programs)
- Lead or participate in care planning meetings
- Assess residents with dementia and refer as needed
- Work with Nurse Care Manager on evidence-based programs
- Coordinate safe discharges and transitions with hospitals and home care
- Advocate for residents and serve as family liaison
- Track metrics and outcomes, prepare reports
Qualifications
- Bachelor's degree with 3-5 years of experience
- Master's in social work or related field and 1-2 years of case management experience in aging preferred
- Passion for working with seniors
- Excellent organizational and interpersonal skills
- Proactive, professional, collaborative, results-oriented
- Positive demeanor, strong communication skills, sound judgment
- Flexible, eager to learn
- Proficient in documentation and software (AASC Online, Windows, Word, PowerPoint, Excel)
- Willing to travel locally in Boston area
- Certification in Dementia Care is preferred
- Fluency in Russian and Chinese is a plus

- Ability to lift, push, pull 25 pounds
- Ability to stand, walk, drive, and sit during shifts
