Position Overview
We are excited to invite a passionate Part Time Registered Nurse (RN) to join our team and support our expanding client network. This role will operate under the supervision of the Director of Patient Services. If you are enthusiastic about home care and thrive in independent work settings, this opportunity is for you!
Key Responsibilities
- Conduct initial RN assessments and evaluations for new clients following LHCSA and NYS DOH guidelines.
- Complete required reassessments every 6 months or as needed to ensure client care is up to date.
- Create and update individualized care plans tailored to meet each client's unique needs.
- Supervise Home Health Aides (HHAs) in the field, including conducting on-site visits and evaluations.
- Ensure compliance with all clinical documentation and regulatory requirements at all times.
- Collaborate with the care team (Care Coordinators, Scheduling, and Family Members) to deliver high-quality care.
- Provide clinical guidance and oversight to field staff.
- Assist in the hiring and training of HHA/PCAs to ensure a strong caregiver team.
Qualifications
- Current NYS Registered Nurse (RN) license in good standing.
- Minimum 1 year of experience in home care, community health, or a related field.
- Knowledge of LHCSA regulations and NYS DOH home care standards is preferred.
- Strong assessment, communication, and organizational skills are essential.
- Ability to work independently and travel locally to clients' homes.
- Comfortable with electronic documentation and the use of mobile devices.
- Must possess a valid driver's license and reliable transportation to visit clients in the Pittsford and greater Monroe County area.
Hourly Wage: $40/hour
