Position Summary
Provides professional nursing care for Apicha Community Health Center patients, adhering to established clinical standards and practices. The purpose of this position is to provide nursing care to patients of all ages, gender, cultures, and background; provide physical and psycho-social support to patients and their significant others; welcome and create an inclusive environment for LGBTQ patients; support and guide members of the clinical team. As a member of the nursing team, work collaboratively with medical providers and all other members of the multidisciplinary team in managing care for a population of patients.
The major duties of the position include, but are not limited to:
Direct Patient Care
Conducts an assessment of patients' physical and psychosocial needs including pain assessment, allergies and objective data.
Maintains reports of patients' medical histories, adheres with protocols, rules and regulations to maintain complete medical records.
Administers prescribed medications, vaccines, and treatments, and reports all critical lab values immediately to the providers.
Assess and implement emergency treatment when needed.
Triage urgent patient care requests (phone, walk-in, portal communication), as well as making appointments when appropriate. Utilize critical thinking, and sound clinical judgment to appropriately determine the next level of care.
Promotes wellness and healthy behaviors through teaching and education of patients and caregivers.
Implement procedures to improve the quality of service and maintain patient flow efficiency.
Adhere to patient confidentiality and Apicha corporate compliance policy.
Collaboration
Works collaboratively with nursing team, addresses issues and concerns with supervisor with a positive problem-solving approach.
Evaluates the effectiveness of the care given by interdisciplinary care team and communicates findings to the team.
Assist with population management by identifying patients lacking preventive health screenings; recommended periodic testing; follow-up appointments (lost to follow-up); treatment or follow-up for actionable test results.
Collaborates with Clinical Nurse Manger to provide seminars and in-house trainings on addressing patient health care concerns, infection prevention and teaching family caregiver's strategies on how to manage their loved ones' conditions.
Compliance
Adhere to patient confidentiality/HIPAA regulations and Apicha corporate compliance policy.
Maintain reports of patients' medical histories, monitors and reports changes in their condition and adheres with protocols, rules and regulations to maintain complete medical records.
Act in a professional manner, reflecting the mission of Apicha Community Health Center demonstrating compliance with all policy, procedures and federal/state regulations as outlined in the Employee Handbook.
Patient Education
Exam Room Oversight
Monitor medications, vaccines and medical supplies and report inventory to Nurse Manager.
Anticipate and oversee preparation of examination rooms with equipment and supplies required for visits. Assists providers with examination and procedures as requested.
Maintains hygienic and safe working environment in compliance with healthcare standards and procedures and adheres to Infection Control efforts.
Self-Development
Participate in ongoing trainings in cultural competency with special consideration including but not limited to underserved populations and the LGBTQ people across the age continuum.
Maintain nursing competence through participation in continuous professional education and the appropriate learning experience.
Qualifications:
Bachelor's degree or Associate of Applied Science in Nursing is required.
2 year's experience with medical surgical unit and/or pediatrics or adult triage preferred.
Must have a current Registered Nurse License in New York State.
Must have the ability to effectively communicate verbally and in writing.
Computer literate, documentation experience with EMR, preferably eClinicalWorks but not required.
Bilingual (preferably Spanish speaking or South Asian or East Asian or South-east Asian).
BLS certified preferred.
