Position: Licensed Social Work Therapist
Location: Woodside, NY
Schedule: Monday–Friday, 7 hours/day (between 8:30 AM – 4:30 PM)
Type: Hybrid (3 days on-site, 2 days remote)
Key Responsibilities:
- Conduct psychosocial and mental health assessments
- Provide individual, group, and family therapy
- Handle crisis intervention and care management
- Maintain accurate and timely documentation
- Facilitate outreach and engagement services
- Participate in interdisciplinary team meetings
- Ensure HIPAA compliance and confidentiality
Requirements:
- LMSW, LCSW, or LMHC (NYS license required)
- Minimum 2 years of clinical experience in mental health
- Bilingual in English & Spanish required
- Experience working with diverse populations
- Knowledge of HIV/AIDS, substance use, harm reduction
- Strong organizational and communication skills
- Proficiency in Microsoft Office and telehealth platforms
