Social Worker (Bilingual English/Mandarin/Cantonese)

The Phoenix Rehabilitation and Nursing Center is seeking a dedicated Social Worker to join our team.

Job Description:

We are looking for a full-time Social Worker who is self-motivated and capable of evaluating and treating residents independently without direct supervision. The ideal candidate should have a passion for working with the geriatric population, be flexible, detail-oriented, and able to multitask effectively.

Qualifications:

• Bachelor's degree in Social Work or a related field; LTC MDS experience preferred.
• Ability to work independently and handle multiple tasks.
• Positive attitude with excellent customer service skills.
• Familiarity with Electronic Medical Records programs, preferably PCC.

Responsibilities:

• Meet with patients/residents and their families to gather information and develop safe, secure discharge plans.
• Complete all necessary forms and documentation promptly as per facility requirements.
• Collaborate with other departments to support patients/residents.
• Make community referrals for discharged patients as needed.

Salary: Up to $68,000 per year
