Job Requirements

• Licensure & Certification:
  • Current State Pharmacist license (active at time of start)
  • Successful completion of educational requirements for state licensure
  • BLS (if required by state)
• Experience:
  • Minimum 2 years recent hospital pharmacy experience
  • EPIC EMR proficiency
• Specialized Clinical Knowledge:
  • Medication reconciliation for discharge
  • Antimicrobial stewardship (e.g., interpreting cultures, managing UTI/pneumonia/SSTI)
  • Pharmacokinetic dosing/monitoring
  • Anticoagulation dosing/monitoring
