Program Quality Liaison On-site | 465 Paul Road, Rochester, NY 14624 Full Time | Monday-Friday Department: Program Operations Pay Range: $27-$30/hour Reports to: Director of Strategy & Innovation for Programs

As a Program Quality Liaison, you'll be the bridge between data and action-supporting safety, compliance, and continuous improvement across all our services.

What You'll Do: Incident & Risk ManagementTrack, document, and analyze program incidents
Identify trends and recommend solutions to reduce risk
Support immediate safety actions and long-term fixes
Coordinate documentation for audits and follow-up plans
Ensure corrective actions are completed on time
Keep teams informed and supported before, during, and after reviews
Monitor Electronic Health Record (EHR) entries for quality and accuracy
Provide feedback, coaching, and resources to staff
Help teams improve documentation and stay aligned with policy
Build reports that turn numbers into narratives-supporting smart decision-making
Monitor key metrics (fire drills, financials, compliance grids, etc.)
Present findings clearly to stakeholders and teams
Act as the go-to resource for compliance communication and updates
Join agency committees (Incident Review, Human Rights) and help spot trends
Use IRMA dashboards to track incidents and support proactive planning
Identify and implement smarter processes
Lead debriefs and coach teams to reduce risk and boost accountability
Promote a culture of learning, safety, and growth
Experience: 2+ years in quality, compliance, or operations (Human Services/Healthcare a plus)
Education: Bachelor's in Human Services or related field preferred; Associate degree with 4+ years experience accepted
Skills:
Strong data analysis and reporting abilities
Clear, professional communication-even with tough topics
Comfortable using EHR systems and data dashboards
Able to collaborate and coach across all levels of the organization
Bonus if you have: IRMA or OPWDD experience, risk management background
