Position Summary:
The Horizon's Community was designed to serve senior residents with Alzheimer's and other dementias in a residential setting. The Horizon's Director will plan, coordinate, and implement programs and activities that provide life-enriching experiences for residents. The Horizon's Director is a part of the management team of the facility.
Qualifications:
- Experience in program and event planning for older adults with memory impairment.
- A minimum of one year of experience working with residents with Alzheimer's and other dementias in a residential setting is required.
- Ability to interact and build relationships with older adults.
- Strong creativity and organizational skills.
- Supervisory experience in a healthcare or service industry.
- Clean driving record and willingness to drive the company vehicle and transport residents.
- Desire to work with older adults.
- Must be 18 years or older. Must have a satisfactory criminal history check.
- Must have a physical exam by a licensed physician. Must have a negative drug screen.
- Must be able to react in an emergency situation.
Primary Responsibilities:
- Oversee the implementation of the Horizon's Life Enrichment Program guidelines.
- Develop, plan, and participate in daily and weekly programming.
- Assess new residents and develop individualized Life Enrichment Plans.
- Create a monthly activities calendar reflecting residents' interests and providing enriching opportunities. Coordinate all special events, including holidays.
- Establish and maintain good relationships and communicate effectively with residents and their families.
- Strengthen community involvement through volunteer promotion among community members. Encourage residents' family members to volunteer and participate in activities.
- Adhere to each resident's Care Plan and notify relevant staff of any changes in condition.
- Ensure all systems for providing and tracking care are completed per Oaks policies and procedures.
- Assist with serving meals and supervise caregiver staff in the dining room. Ensure the dining area is cleaned after each meal.
- Maintain supplies for activities within budget guidelines.
- Arrange transportation for outings.
- Perform other duties as assigned by the Executive Director.
- Comply with policies, training, and regulations.
General Management:
- Train and coach care employees in Memory Enhancement and Life Enrichment Programs.
- Supervise caregiver staff to ensure quality care and adherence to philosophy and policies.
- Develop staffing schedules to meet residents' needs.
- Ensure care documentation is current.
- Review communication tools used in care daily.
- Communicate residents' needs during shift changes and with leadership as needed.
- Participate in the Manager-on-Duty rotation.
- Lead by example, promote teamwork, and provide ongoing coaching.
- Maintain an open-door policy for staff concerns.
- Respond to injuries according to policies.
- Assist in training care employees per regulations.
Physical Job Requirements:
Employees must perform key physical tasks for extended periods. Proper body mechanics and safety measures are essential when supporting or transferring residents. In an eight-hour day, employees will:
