CCBC and the Department of Mental Health (DMH) will work collaboratively in the PACT- Y program to meet the needs of youth and families who want a creative, flexible, dynamic team to support them in achieving their goals. As a Mentor/Case Manager you will work within a multi-disciplinary team to improve the social, emotional and recreational functioning of youth as they transition to adulthood. The Mentor/Case Manager will provide opportunities in the community for youth to practice skills, assist with obtaining jobs, explore after-school activities and support goals identified on the team treatment plan.Responsibilities
