QUALIFICATIONS for the Licensed Practical Nurses LPN Position:
- Graduate of state-approved school with course in pharmacology
- Current license in the State of Pennsylvania
- Minimum of two [2] years experience in patient care
- Reliable, independent means of transportation to make home visits. Drivers must have a valid driver's license and drive a vehicle with insurance limits as established by the state.
DUTIES AND RESPONSIBILITIES for the Licensed Practical Nurses LPN Position:
- Provides skilled nursing care that includes activities such as teaching and treatments (including IV therapy- if IV certified, venipuncture and complex wound care) and health promotion practices
- Executes preventive, rehabilitation, and restorative nursing procedures appropriate for the patient's care and safety
- Assesses signs and symptoms in conjunction with reporting reactions to treatments, including drugs, changes in patient's physical, mental, or emotional condition to the physician and the supervising Registered Nurse
- Evaluates the patient and home situation to determine the health teaching required at each skilled visit.
- Evaluates the patient condition and home situation to determine what assistance will be available from the designated representative in caring for the patient.
- Instructs and educates the patient and the designated representative regarding the care requirements of the patient in the home.
- Conducts a comprehensive physical examination and nursing assessment of the patient at each skilled visit.
