Description The Kentucky Department for Community Based Services (DCBS) is a multi-faceted agency that provides servicesand programs to enhance the self-sufficiency of families; improve safety and permanency for children andvulnerable adults; and engage families and community partners in a collaborative decision-making process. Weoffer an opportunitywork with individuals and families applying for public assistance benefits in the Commonwealth. DCBS is searching for a Case Management Specialist I who is highly skilled and motivated to help others. Thisincludes providing professional services directly to the public in reviewing, monitoring and determiningcontinued eligibility for assistance programs. The Case Management Specialist I works with families to assesstheir needs in order to achieve self-sufficiency. Attention to detail, experience communicating with thepublic, and the ability to track deadlines are all skills necessary for this position. The Case ManagementSpecialist I is a demanding and rewarding position that requires leadership, maturity and a strong knowledgeof policy. The position you are applying for has access to or use of federal tax information (FTI) or meets the statutorydefinition of front-line staff. Therefore, pursuant to IRS Publication 1075 and 900 KAR 1:009 or KRS 194A.062and 900 KAR 1:050, applicants for this position in the Cabinet for Health and Family Services (CHFS) shallsubmit to a fingerprint-based criminal background check by the Department of Kentucky State Police and theFederal Bureau of Investigation.   • Assessment of family needs and explanation of agency services.
  • Develops employment focused transitional assistance plan designed for each family specific to their needs.
  • Screens for risk factors and barriers.
  • Identifies resources and makes appropriate referrals while coordinating services.
  • Provides supportive services to families such as childcare and transportation.
  • Assists clients with exploration of work opportunities.
  • Makes referrals to various agencies/employers for job readiness, job development and job placement.
  • Completes forms and gathers required documentation to determine eligibility.
  • Maintains ongoing records of family plans/activities and monitors progress of families toward self-sufficiency.

  • Willingness to work as part of a team and collaborate with families and community partners.
  • Strong verbal and written communication skills.

EXPERIENCE, TRAINING, OR SKILLS:NONE Substitute EDUCATION for EXPERIENCE:NONE Substitute EXPERIENCE for EDUCATION:Experience in social work, counseling, providing direct services to individuals and/or families (i.e., employment services, community action agencies) or experience in reviewing, monitoring, determining eligibility for, and/or administering Supplemental Nutrition Assistance Program, Kentucky Transitional Assistance Program, Medical Assistance, Child Support, or other public assistance programs will substitute for the required education on a year-for-year basis. Current or prior military experience will substitute for the required college on a year for year basis.
