Post Internal Days 0 Category Preventative Services Description POSITION TITLE: Case AideDEPARTMENT: Family Support ServicesPROGRAM: PreventiveREPORTING RELATIONSHIP: Unit SupervisorMAJOR DUTIES AND RESPONSABILITIES
Makes home visits designated on the basis of safety reviews. Makes school visits to collect educational information and report cards. Provides outreach to the family. Assists parents in obtaining needed services through advocacy and other interventions. Acts as escort for clients to Housing Court, Public Assistance, Medicaid and other public entitlement offices, as well as to service programs. Collaborates in the agency's establishment and development of linkages with relevant community services. Works collaboratively with the social worker and case supervisor. Participates in case planning conferences. Attends specific training for working with families and children. Responsible for providing emergency case coverage as needed. Provides referrals to child/family as needed (ie: chemical abuse, medical or mental health, recreational) from a community based perspective. Provides the necessary documentation to support casework services in a timely fashion, including but not limited to Progress Notes. Other duties as assignedQUALIFICATIONS BA with work experience required Experience in working with families and communities. Bi-lingual (Spanish), preferred NYS Driver's license preferredWORK SCHEDULEFlexible schedule required.
