Summary
Provides clinical pharmacy services to ambulatory patients. Provides comprehensive medication management in multiple areas of practice, including but not limited to anticoagulation, perioperative management of anticoagulants and antiplatelets, anemia management with erythropoiesis stimulating agents, diabetes, and hypertension. Ensures departmental efficiencies and compliance with regulatory bodies. Instructs other pharmacists, pharmacy residents and pharmacy students.
The Medication Management Clinic provides comprehensive pharmaceutical care and care coordination to ambulatory patients who are on high-risk medications or who require disease state management.
